Your application will be complete when we have received the following items.
If the applicant is a sibling of a current student, please call our Admissions Office for deadlines and process information. High school applicants please visit our High School website for admissions information.

• $60 Application Fee
• Photo of Student Applicant
• Confidential Parent Questionnaire
Grades 2–8 Applicants Only:
• Request for Transcripts*
• Copies of Two Most Recent School Report Cards
• Copies of any Relevant Educational Assessments
Grades 4–8 Applicants Only:
• Teacher Evaluation Form (PNAIS)*
Due Friday, February 3, 2012
• SSS Financial Aid Application Submitted Online.
You may also request a mail-in form from the Admissions Office
Due Friday, February 10, 2012 • 4:00 pm
• Completed financial aid application, including your
IRS 2011 1040, all schedules and W-2's
Financial Aid Form for New Applicants
Financial Aid Form for Returning Families
Due Monday, March 26, 2012 • 4:00 pm
• Response due today from all accepted applicants,
including their commitment form, deposit and new student fee.
Admissions decisions will be mailed March 15, 2012.
Early Childhood Program class assignments will be
mailed by June 1, 2012.
*These forms need to be processed by your child’s current school or teacher, then sent to SWS. As this process may take some time, we advise that you send them to the appropriate party early in your application process.
